How to use it:

Once the Health & Medical questionnaire is opened, go to your browser menu and “save as” to your computer. Then open the saved file, fill out all appropriate fields and save it. Send the saved completed document back to us as an attachment to helen@pilatesinsevenoaks.co.uk.
